A care co-ordinator is a valuable member of the primary care team. The role provides extra time, capacity and potentially expertise to a practice team and so can greatly improve the care provided. Care co-ordinators work with individuals to provide holistic support.
Health Visitors
A health visitor is a registered nurse who provides families with children under five years old with support and advice around the general aspects of mental, physical and social wellbeing. Please contact the surgery if you do not have a health visitor.
Social Prescribers

We have a dedicated PCN Social prescriber linked to our practice, who helps bridge the gap between local agencies and general practice. Link workers give people time, focusing on ‘what matters to me’ and taking a holistic approach to people’s health and wellbeing. They connect people to community groups and statutory services for practical and emotional support. First Contact Physiotherapist (FCP)

Our First Contact Physiotherapists (FCPs) are highly trained to evaluate and manage musculoskeletal (MSK) problems. They can assess your condition, provide a diagnosis, and recommend appropriate exercises or further investigations.
If you are experiencing an MSK issue, please consider requesting an appointment with our FCP instead of a GP. You can do this by contacting our reception team.
